*This form is due on April 1. If you have an extenuating circumstance preventing you from meeting this deadline, you must contact Ms. Rachel at rickes@tntech.edu as soon as possible.

*All Honors students must fill out this form even if they're not requesting an Honors permit for designated option(s).

*All Honors-credit options below require a permit to register.

*You must earn Honors credit each semester until you complete all 22 required credits in order to remain eligible for Honors benefits (such as early registration).

*If you select an option that requires a proposal, the completed, signed proposal is due on April 15.
