Event Registration Form

Instructions:
1. A chapter must submit an Event Registration Form at least five business days before hosting an event for review and approval by the Office of Greek Life and the chapter's faculty advisor.
2. If two chapters are cohosting an event, then each chapter must submit an Event Registration Form before the event can be approved.
