I hereby apply for a SGA Loan in the amount listed above to assist in the payment of my educational expenses while enrolled (must be confirmed for the term I'm requesting this loan for).

In order to assist in processing this loan, I am listing below the names and addresses of two personal references (must have separate, complete addresses) who will be listed as the co-signer of my Promissory Note. Your Co-Signer must be able to present a valid government ID to verify U.S. Residency.

I understand that the University may disclose to a credit bureau both the disbursement of this loan as well as the default of this loan.

I agree that any financial aid received after this loan is processed will be used to repay this loan.

I understand and accept that if I fail to make timely payments of my financial obligation to the Institution or fail to make acceptable payment arrangements to bring my account current, the Institution may refer my delinquent account to a collection agency. I further understand that I may be responsible for paying the collection agency fee, which may be based on a percentage at a maximum of 33 1/3 % of my delinquent account, together with all fees and expenses, including reasonable attorney’s fees, necessary for the collection of my delinquent account. Finally, I understand that my delinquent account may be reported to one or more of the national credit bureaus.

All parties and endorsers hereby expressly waive demand, protest and notice of nonpayment.
